Do I need to fill out forms before my first visit?
Completing your forms in advance is recommended. It saves time at check-in and gives Dr. Hassey more time to focus on you.
Which form should I use?
Patients 18 and older should use the Adult New Patient Form. Parents or guardians should complete the Child New Patient Form for patients under 18.
How do I submit my completed forms?
Bring your completed forms to your appointment at our Carlsbad or La Costa office, or call (760) 434-2288 - Carlsbad Office or (760) 434-2288 - La Costa Office for other submission options.
What is the AutoPay form?
The AutoPay form sets up automatic credit card payments for your treatment plan. Complete it and email it to caminorealortho@gmail.com.
What if I can’t open a PDF form?

What else should I bring to my first appointment?
Please bring your insurance card, a list of current medications, and a photo ID. A parent or guardian should accompany minors.
